Almost every IV menu in Cabo lists “B-complex” near the top. The marketing makes it sound like an energy drink. The medical use is more interesting — and narrower. Here’s what B-complex actually delivers, when a physician adds it, and which claims to ignore.

What’s in B-complex

A typical IV B-complex includes:

  • B1 (thiamine): energy metabolism; deficiency causes Wernicke’s encephalopathy in heavy alcohol users.
  • B2 (riboflavin): cellular energy; some evidence in migraine prevention.
  • B3 (niacin): energy metabolism; high doses cause flushing.
  • B5 (pantothenic acid): energy and steroid hormone production.
  • B6 (pyridoxine): neurotransmitter and red-blood-cell production; interacts with levodopa.
  • B7 (biotin): can interfere with lab thyroid testing.
  • B9 (folate / folic acid): DNA synthesis and red-blood-cell production.
  • B12 (cyanocobalamin or methylcobalamin): neurological function and red-blood-cell production.

Most clinical IVs use a pre-mixed pharmaceutical preparation; some clinics offer “designer” combinations with specific doses.

The legitimate clinical uses

  • Acute thiamine repletion in alcoholic patients or severe malnutrition — the standard “banana bag” given in ERs.
  • Treatment of confirmed B12 deficiency when oral isn’t reliable (pernicious anemia, gastric bypass, severe deficiency).
  • Migraine prevention in some patients with regular B2 (riboflavin) dosing — IV not usually the right route here; oral works.
  • Wernicke encephalopathy prevention when giving glucose to someone with low thiamine — emergency use.
  • Severe gastroenteritis with significant fluid loss — included as a small “support dose” in the IV.

The marketing claims that don’t hold up

  • “Energy boost”: only if you were deficient. In a healthy adult with normal levels, IV B-complex doesn’t give you measurable extra energy — your kidneys just excrete the excess.
  • “Detox”: there’s no specific detox mechanism.
  • “Anti-aging” or “skin”: no clinical evidence for IV B-complex specifically driving these outcomes.
  • “Cure” for hangovers: B-complex helps mildly with hangover symptoms because chronic alcohol use depletes thiamine. It’s not a cure.

The reality: B-complex is a useful adjunct in many IVs and a specific treatment in a few situations. The marketing hype is louder than the science.

B12 specifically

If you have a documented B12 deficiency at home and your prescribing doctor has you on injections, a Cabo B12 shot can substitute on schedule while you travel. If you’re not deficient, the boost is short-lived. We can also do an IM (intramuscular) B12 shot as an add-on without a full IV.

Who should not get high-dose B-complex

  • Anyone on levodopa (Parkinson’s) — B6 reduces levodopa effectiveness.
  • Severe kidney disease — needs dose adjustment.
  • Allergy to any B-vitamin component.
  • Patients about to have lab work — biotin in some B-complex preparations can throw off thyroid tests.

Side effects

Generally mild: flushing (from niacin), mild stomach upset, occasional rash. Most patients have no side effects at standard IV doses.

Frequently asked questions

Will B-complex give me an energy boost?

If you’re deficient, yes. If you’re not, the effect is mild and short-lived.

Is IV better than oral B-complex?

For documented deficiencies or after severe gastrointestinal loss, yes. For routine supplementation in a healthy adult, oral is just as effective and a fraction of the cost.

Can I get just B12, not the whole complex?

Yes — as an IM injection add-on or as part of a customized IV. Ask at intake.

Is B-complex safe in pregnancy?

Most of the B-vitamins are safe at standard doses, and folate is actually recommended in pregnancy. High-dose B6 has specific cautions; the doctor will adjust.
